I am an in network provider for Horizon.
Ask your insurance company the following questions:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is the deductible and has it been met?
- Does my plan limit the number of sessions per calendar year? If so, what is the limit?
I accept cash and check as forms of payment.
